Aetna Launches PCMH

In an effort to provide continuous, comprehensive, and coordinated care to its patients, U.S. health insurer Aetna Inc. (AET - Free Report) has formed its Patient-Centered Medical Home (PCMH) program in Massachusetts.
The PCMH is a model of healthcare based on an ongoing, personal relationship between a patient, doctor and the patient's care team. Whether the medical needs primary or secondary, preventive care, acute care, chronic care, or end-of-life care – the PCMH model ensures the patient has a medical "home" -- a single, trusted doctor and care team, through which continuous, comprehensive and integrated care is provided.
Aetna’s PCMH will provide the following services: meeting the large majority of each patient’s physical and mental health care needs, including prevention and wellness, acute care, and chronic care; respecting each patient’s unique needs, culture, values, and preferences; care coordination across all elements of the broader health care system, including specialty care, hospitals, home health care, and community services and supports; providing accessible services with shorter waiting times for urgent needs, enhanced in-person hours, around-the-clock telephone or electronic access to a member of the care team; and  a commitment to quality and quality. 
Under Aetna’s PCMH program primary care, providers who are not engaged in the company’s other incentive programs will provide necessary service. Their fees will be based on the quality of work performed and not on quantity. 
PCMH is a big part of health care reform efforts across the country. Insurers are aligning their processes and staff to identify and engage high impact, high value primary and secondary care providers, services and care delivery partnerships that improve quality and outcomes of care while lowering medical cost trends. 
Health insurer WellPoint Inc.’s subsidiary Amerigroup is also activey engaged in PCMH initiative.
