UnitedHealth Group Incorporated (UNH - Free Report) outlined improving operating momentum during its third-quarter 2026 earnings call, emphasizing disciplined medical cost management, business modernization and a return to sustainable earnings growth in 2027.

Management raised its full-year earnings outlook while acknowledging persistent commercial medical cost pressures, uneven Medicaid funding and continued investments across Optum.

UNH Raises Outlook and Signals 2027 Growth

Chief financial officer Wayne DeVeydt raised the 2026 adjusted earnings guidance to at least $20.50 per share, establishing a stronger earnings base heading into 2027.

DeVeydt said the lower end of UnitedHealth's long-term 13%-16% earnings growth target represents a reasonable starting point for 2027, with current analyst consensus potentially understating the opportunity.

Third-quarter adjusted earnings of $4.69 per share exceeded the Zacks Consensus Estimate of $4.12. Revenues of $112.69 billion surpassed the consensus estimate of $111.38 billion.

UnitedHealth Faces Elevated Commercial Costs

UnitedHealthcare CEO Timothy Noel emphasized that commercial medical cost trends remain above the company's initial 11% expectations, reflecting specialty drug expenses, higher clinician costs and No Surprises Act arbitration pressures.

During questioning from UBS, Noel acknowledged that elevated utilization would extend the timeline for achieving long-term commercial margin objectives.

Responding to Bank of America, Noel clarified that management was not seeing medical costs accelerate. Instead, commercial trends were stabilizing, while government programs showed moderation.

UNH Targets Medicare and Medicaid Margin Recovery

Bobby Hunter, CEO of Government Programs and Head of Medicare Insurance, told Wolfe Research that Medicare margins should finish 2026 near the upper end of the company's 2%-4% target range.

For 2027, Hunter highlighted growth opportunities in Dual Special Needs Plans, Chronic Special Needs Plans and high-value HMOs. Management continues refining its Medicare Advantage product portfolio while maintaining pricing discipline.

Medicaid CEO Michael Cotton said full-year core Medicaid margins should improve to moderately better than negative 1%. He identified clinical cost management and state funding adjustments as recovery drivers, with a goal of returning to target margins by 2028.

UnitedHealth Advances Optum Health Turnaround

Optum Health generated third-quarter operating earnings of $735 million compared with $41 million a year earlier, reflecting improved medical cost management and operational discipline.

Optum Health CEO Krista Nelson highlighted opportunities in accountable care, home health, hospice and post-acute services. The business remains focused on progressing toward its 6%-8% operating margin target by 2028.

Responding to Barclays, Nelson explained that fourth-quarter performance would reflect normal risk seasonality and additional technology investments. She emphasized that operational improvements achieved during 2026 should continue supporting performance.

UNH Expands Pharmacy and AI Opportunities

Optum CEO Patrick Conway emphasized integrating pharmacy services with medical care, including specialty pharmacy, behavioral health pharmacy and infusion services.

During JPMorgan's questioning, Chief Growth Officer Jon Mahrt reported specialty pharmacy volume growth of 2.6% despite a broader quarterly volume decline. He also highlighted customer retention in the high 90s within pharmacy benefit management.

Chief Digital and Technology Officer Sandeep Dadlani described Optum Insight's multiyear AI transformation. Its Integrity One platform expanded from two implemented clients at year-end 2025 to 15, with another 24 implementations underway.

UnitedHealth Prioritizes Sustainable Execution

CEO Stephen Hemsley emphasized restoring financial discipline, simplifying healthcare administration and strengthening relationships with clinicians and consumers.

The company expects AI-enabled processes, integrated value-based care and operational modernization to support productivity and service improvements while positioning its businesses for sustainable growth.

Management plans to provide a more comprehensive 2027 outlook before year-end, including additional clarity on business-level opportunities and investments.
