Why ISRG Is Lowering Instrument Costs Despite Strong Profitability

Read MoreHide Full Article

Key Takeaways

  • ISRG plans to extend EndoWrist instrument life in 2027 to lower costs for select robotic procedures.
  • ISRG's I&A revenues rose 18% to $1.73B in Q2, but longer instrument use could pressure revenue per procedure.
  • ISRG's 42% adjusted operating margin supports its strategy to expand adoption through lower costs.

Intuitive Surgical (ISRG - Free Report) is preparing to lower customer costs for selected robotic-assisted procedures through its Extended Use Program (“EUP”), reflecting a strategic trade-off between near-term instrumentation and accessories (I&A) revenues and long-term market expansion.

Beginning first-half 2027, the company plans to increase the useful life of a subset of EndoWrist instruments compatible with its fourth- and fifth-generation da Vinci platforms. The initiative targets benign, high-volume procedures where affordability can influence adoption, particularly in cost-sensitive geographies and healthcare systems with lower reimbursement rates.

The strategy could put pressure on I&A revenue per procedure as customers extend the use of eligible instruments before replacing them. In the second quarter of 2026, I&A revenues rose 18% year over year to $1.73 billion, while da Vinci I&A revenue per procedure reached approximately $1,830, compared with $1,800 a year earlier.

Management is yet to quantify EUP’s financial impact, with further details expected following its analysis and pricing decisions. The rollout is expected to have a gradual impact throughout 2027 rather than result in an immediate, one-time change.

However, Intuitive Surgical is prioritizing growth and profitability together, and believes lower procedure costs can unlock incremental demand. By lowering the cost of robotic surgery for selected benign procedures, EUP could encourage hospitals and physicians to increase utilization, particularly in markets where cost has limited adoption. Higher utilization and a broader customer base could strengthen the company’s installed ecosystem and support recurring revenue opportunities over time.

The initiative comes as Intuitive Surgical remains profitable, reporting a 42% adjusted operating margin in the second quarter. Its broader strategy is to use engineering innovation, manufacturing scale and cost reductions to extend access without compromising product quality or reliability. The near-term trade-off impact will likely lead to slower I&A revenue growth per procedure while driving the longer-term opportunity for a larger market share and higher system utilization.
