BrightSpring Health Services, Inc. (BTSG - Free Report) raised its 2026 outlook and reaffirmed its expectations for organic earnings growth next year, supported by specialty pharmacy demand and home health expansion.

Adjusted EPS of $0.54 beat the Zacks Consensus Estimate of $0.49, while revenues of $4.21 billion exceeded the $3.86 billion estimate. Analysts focused on whether the growth drivers can endure.

BrightSpring Lifts Guidance and Frames 2027

Chief financial officer Jennifer Phipps raised full-year 2026 revenue guidance to $15.9-$16.125 billion and adjusted EBITDA guidance to $850-$860 million. The outlook excludes Community Living and acquisitions not yet closed.

The forecast includes roughly $35 million in adjusted EBITDA from acquired Amedisys and LHC branches and indicates revenue growth of 23.2-24.9% and adjusted EBITDA growth of 37.6-39.3%.

Chief executive officer Jon Rousseau reiterated expectations for at least 15-20% organic adjusted EBITDA growth in 2027. Phipps expects broadly similar year-over-year quarterly increases, with slightly stronger growth in the second half.

BTSG's Drug Launches Sustain Specialty Volume

Specialty and Infusion revenues climbed 34% to $3.2 billion, supported by 29% prescription growth. Rousseau said seven limited-distribution drugs launched during the quarter, bringing year-to-date launches to 19 and supported LDDs to 162.

A Bank of America analyst asked about the potential contribution of a significant new therapy. Rousseau declined to discuss individual drugs but said 2026 launches are outpacing the 2025 cohort in their first year.

A TD Cowen analyst pressed on the sustainability of prescription growth. Rousseau pointed to manufacturer relationships and a continuing pipeline, with preparation typically beginning at least a year before launch.

BrightSpring Manages Pharmacy Headwinds

Home and Community Pharmacy revenues declined 6% to $553 million. Phipps attributed the decrease to roughly $50 million of Inflation Reduction Act effects and the exit of uneconomic customers.

She projected another $50 million reimbursement impact in the fourth quarter, bringing the full-year effect to approximately $200 million. Rousseau expects an IRA headwind in 2027 and stressed efficiency, logistics and cost per fill.

Asked by a Leerink Partners analyst about gross profit per prescription, Phipps identified specialty mix as the main sequential driver. She prioritizes total gross profit and EBITDA over per-prescription metrics.

BTSG Expands Home Health and Primary Care

Provider Services revenues increased 31% to $481 million, while home health average daily census rose 58%. Phipps said acquired Amedisys and LHC branches contributed approximately $76 million in quarterly revenue; excluding them, home health revenues grew 12%.

Rousseau said that integration is proceeding as expected and 96% of legacy home health branches carry ratings of at least four stars. He also discussed extending virtual primary care to homebound and higher-risk patients.

Responding to a BTIG analyst, Rousseau discussed the Medicare LEAD ACO program beginning in January 2027. He identified 2028 as a period for potentially meaningful economics and said the company's current arrangements entail no downside risk.

BrightSpring Keeps Acquisitions Selective

Operating cash flow reached $204 million during the quarter, while leverage fell to 1.79 times from 2.15 times in June. Phipps anticipates at least $700 million of 2026 operating cash flow excluding Community Living transaction effects.

A UBS analyst asked how the stronger balance sheet affects acquisitions. Rousseau favored tuck-in transactions and slightly larger deals involving around $20 million of EBITDA or less.

He stressed diligence despite an extensive pipeline and disavowed partnerships for dealmaking. Phipps also acknowledged some one-time working-capital benefits behind cash flow, while maintaining a longer-run operating cash conversion expectation above 60% of adjusted EBITDA.

BTSG Focuses on Execution Into 2027

Rousseau portrayed the quarter as evidence of broad-based execution across pharmacy and provider operations, emphasizing service quality and coordinated care rather than a single growth engine.

Phipps said that technology and AI initiatives rolling out in 2026 should contribute more benefits in 2027. Management continues to prioritize efficiencies, disciplined capital deployment and investment across service lines.
