  • Ingrezza sales rose 17% to $801 million, while Crenessity revenue surged 115% to $211 million.

Neurocrine Biosciences (NBIX - Free Report) reported third-quarter 2026 adjusted earnings per share (EPS) of $3.29, up 51.6% year over year. The figure beat the Zacks Consensus Estimate of $2.54, reflecting strong product demand and operating leverage.

Total revenues climbed 41% year over year to $1.12 billion, surpassing the Zacks Consensus Estimate of $1.04 billion. Record Ingrezza prescriptions and growing contributions from Crenessity and Vykat XR fueled the performance.

Shares of Neurocrine rose 6% in after-hours trading on Monday, likely due to the better-than-expected earnings results and raised product revenue guidance for Ingrezza. The gains extended into Tuesday's pre-market trading.

Year to date, the stock has gained more than 2% against the industry’s nearly 4% decline.

NBIX's Ingrezza Franchise Anchors Growth

Ingrezza net product sales increased 17% year over year to $801 million from $687 million. Sales beat the Zacks Consensus Estimate of $736.5 million, supported by record new and total prescriptions and high patient persistence.

The quarter included a $20 million benefit from customer order timing, which management expects to reverse in the fourth quarter. Even excluding this benefit, the franchise benefited from rising demand across psychiatry, neurology and long-term care. Management expects further gains from its expanded sales force.

Neurocrine’s Crenessity Launch Gains Traction

Crenessity generated $211 million in net sales, up 115% from the prior-year quarter. The figure topped the Zacks Consensus Estimate of $200.4 million, as steady new patient additions and strong treatment persistence sustained uptake.

Approximately 80% of dispensed prescriptions were reimbursed. More than 15% of the estimated diagnosed population with classic congenital adrenal hyperplasia (CAH) has been prescribed the medicine. Management noted expanding adoption among adult endocrinologists, with adults now representing roughly half of treated patients.

NBIX's Vykat XR Adds to Product Revenues

Vykat XR posted $107 million in third-quarter sales, reflecting sequential growth driven by new patient demand. Neurocrine added the treatment for hyperphagia in Prader-Willi syndrome (PWS) through its May 2026 acquisition of Soleno Therapeutics.

Management said recent external events could delay the drug's expected growth trajectory. Following questions about post-marketing safety reports, executives emphasized patient selection, dose titration and ongoing monitoring. Neurocrine also plans a 2027 study involving PWS patients and complex health conditions.

Neurocrine's Higher Expenses Temper Profitability

Adjusted research and development (R&D) expenses increased 39% to $317 million, reflecting investment in expanding clinical programs.

Adjusted selling, general and administrative (SG&A) costs rose 26% to $326.6 million, driven by sales force expansion and support for Vykat XR.

NBIX Raises Its 2026 Ingrezza Guidance

Neurocrine increased its full-year forecast for Ingrezza sales to $2.90-$2.925 billion from $2.825-$2.875 billion. The revision reflects demand strength despite the expected reversal of favorable customer order timing in the final quarter.

The company lowered its adjusted SG&A expense guidance to $1.29-$1.31 billion from $1.325-$1.350 billion and maintained adjusted R&D guidance at $1.14-$1.19 billion.

For 2027, management expects double-digit Ingrezza volume growth to offset mid- to high-single-digit net pricing headwinds.

Neurocrine’s Pipeline Targets 2027 Milestones

The company anticipates phase III data in 2027 for osavampator in major depressive disorder and direclidine in schizophrenia. A Friedreich's ataxia gene therapy program is also expected to enter Phase I in the coming months, with initial patient-level data anticipated in 2027.

Neurocrine recently initiated a phase II study of NBI-'567 as an adjunctive treatment for cognitive impairment in Alzheimer's disease. In obesity, the company started Phase I testing of NBIP-'1968, a triple-receptor agonist, alongside its existing Phase I CRF2 agonist program, NBIP-'2118.

NBIX's Cash Generation Supports Investment

Neurocrine ended September with approximately $790 million in cash, cash equivalents and marketable securities compared with $2.543 billion at the end of 2025. Management said the quarter generated approximately $400 million in operating cash flow.

The company also deployed about $100 million to repurchase shares and offset dilution from stock-based compensation. Management's capital allocation priorities remain commercial expansion, pipeline development and disciplined business development.
